This Project Scope of Work was for the inspection of suspect asbestos containing building materials as required for the application of a demolition permit. The inspection services included, but were not limited to, the following items:
** Sampled all suspect asbestos containing building materials in accessible areas
** Sampled approximately 95 suspect materials ** Took corresponding photograph for each sample and designated the location on a blueprint
** Prepared a written report with all positive samples identified, a chart of ALL samples in Excel, sample photographs, and location, description, condition and potential for disturbance noted in the report
** Signed off on the Demolition Permit

This Project Scope of Work was for the inspection of suspect lead based or containing paint as required by HUD. The inspection services included, but were not limited to, the following items:
**Sampled all paint in the identified areas to be disturbed during remodel – all colors and layers sampled
** Sampled approximately 20 locations ** Took corresponding photograph for each sample and designated the location on a drawing
** Prepared a written report with all positive samples identified, a chart of ALL samples, sample photographs, and location, description and condition noted in the report
** Corresponded with client for best remediation methods (risk assessor)

Project 3 – Johnson Residence

This Project Scope of Work was for the inspection/assessment of potential mold contamination in a residence. Air monitoring as well as wipe samples were collected for analysis. The DOHMH Section 2.2 Guidelines on Assessment and Remediation of Fungi in Indoor Environments was used as a guide as recommended by CDPHE. A copy of which is available for review at our office for any of our clients. The inspection services included, but were not limited to, the following items:
** Sampled three areas in the home and a control sample was taken outdoors
** Four (4) air samples and One (1) wipe sample was taken and submitted to the lab for analysis
** Took corresponding photograph for each sample and designated the location on a drawing
** Prepared a written report with particles identified; fungal elements and descriptions; healthy building      considerations list
** Corresponded with client for with any questions and/or concerns

This Project Scope of Work is for the inspection of suspect asbestos containing building materials as required for the update of the management plan(s) for the two elementary schools. The inspection and management plan services will include, but are not limited to, the following items:
** Identify all materials sampled in previous inspections
** Note any suspect materials missed and sample all suspect asbestos containing building materials in        accessible areas
** Organize and reassemble an acceptable management plan(s) according to CDPHE and AHERA guidelines
** Communicate with the Local Education Authority (LEA) during all phases of work
** Communicate with the maintenance personnel about training and maintenance of asbestos materials

This Project Scope of Work was for a full Phase I Environmental Site Assessment of 4 City Blocks located in Pueblo, Colorado. The Phase I ESA consisted of the following:
** Guideline: ASTM Standard for Commercial Real Estate (1527-99)
** Regulatory Agency Review -EDR
** Historical Research – Title Search, Plat Maps, City Directories, Assessors Cards, Aerial Photographs
** Interviews – Present and Past Owners/Operators, Fire Department, Health Department
** Maps – Topography and Site Location
** Digital Photographs
** Unique Phase I as there were several businesses including two car dealerships, a hotel, a restaurant, a       vacant lot, an auto parts store, a health food store, and a transmission repair shop
** Coordination with all property owners and report was finished in 10 working days
** Written Report – Data, Photos, Maps and Findings and Conclusions

This project is for review of the current SPCC plan. SPCC plans are required by Title 40, Code of Federal Regulations Part 112 (40 CFR 112). The noted rules apply to all petroleum-based fluids at a facility but not to other fluid storage. The SPCC can be combined into a comprehensive so called “Integrated Contingency Plan (ICP).” The client wants the SPCC plan and update.

** Walk of the plant and inventory of tanks and contents

Project 8 – US EPA Region VIII

Development of Monitoring Well 13 wells on Site Former LUST site On Indian Territory Compliance and Communication with Indian Reservation Regulations
|

This Project Scope of Work was for the site characterization of a LUST release at the Standing Rock Gaming Authority, Ft. Yates, North Dakota. The Site Characterization consisted of the following:
** Tank removal, including removal of the foundation of building ** Subsurface soil sampling and field monitoring
** Installation of temporary (15) and subsequently permanent monitoring wells (13)
** Removal of soil from tank pit to a soil farm located on Site
** Interpretation of results as presented to the EPA
Chemicals of concern include: TEPH (Diesel), TVPH, MTBE, BTEX, Naphthalene, and 1,2 DCA APEC supplies the EPA with all documentation concerning the groundwater elevations, the well locations and the contour intervals. APEC conducts all well development and preliminary sampling.

** Installation of permanent monitoring wells ** Development of Corrective Action Plan
** Proposed In-Situ Bioremediation
** Reimbursement clients’ costs from the OPS UST fund
